Medical Absence Form
Please complete the form to inform us of the date, time and brief explanation of your child's planned absence. Once received, we will ask you to provide medical evidence for this appointment. Please ensure that you add your child's name and class. If the appointment is in the morning and your child requires a school lunch, please include their lunch option.
Holiday Absence Form
Please include your child's name and class along with holiday dates. Please note: All holidays are unauthorised. Legislation makes it clear that head teachers may not grant leave of absence during term time. They no longer have the discretion to allow holidays up to 10 days or to agree extended leave for parents to visit their country of origin (This is stated by the Director of Education and Early Intervention). If you believe you have exceptional circumstances please speak to Samantha Field, our parent officer.
Other Absence
As a parent/carer you should fill in this form if you want to take your child out of school during term time for reasons other than holiday or medical appointments. (For example: Parent medical reasons, funerals, visits to secondary schools etc..)
If you feel there are exceptional circumstances please also email our parent officer after submitting the form with any extra information.
