Please complete the form to inform us of the date, time and brief explanation of your child's planned absence. Once received, we will ask you to provide medical evidence for this appointment. Please ensure that you add your child's name and class. If the appointment is in the morning and your child requires a school lunch, please include their lunch option.
If you feel there are exceptional circumstances, please also email our parent officer after submitting the form with any extra information.
All other non-urgent/routine appointments should be made out of school hours.
Not all requests are authorised by the school.
